“KVOO Black Box”
OFFICIAL RULES
 
NO PURCHASE NECESSARY TO ENTER OR WIN.  A PURCHASE WILL NOT INCREASE YOUR CHANCE OF WINNING.
1.         PROMOTION DATES
a.         These rules govern the KVOO “Black Box” contest September 2012 promotion (the “Promotion”) being conducted by KVOO(the “Station”) beginning at approximately 9:12AM Central Time (“CT”) on September 24, 2012 and ending at approximately 5:12PM CT on November 30, 2012, excluding  Thanksgiving Day November 22nd and the following Friday November 23rd, 2012. 
2.         ELIGIBILITY RESTRICTIONS
a.         The Promotion is open to all legal residents who are 18 years of age or older and who reside in the Greater Tulsa Area. Employees of KVOO (the “Promoter”), its corporate parent, subsidiary, and affiliated entities, its advertising agencies, participating sponsors/promotional partners, and the members of their immediate families (spouse, parents, siblings or children) or households are ineligible to participate or win.  This Promotion is subject to all applicable federal, state, and local laws and regulations.  Void outside the United States and where prohibited. 
b.         Only one winner per household is permitted in any Promoter contest.
c.           Entrants are required to provide truthful information and Promoter will reject and delete any entry that it discovers to be false or fraudulent.  Promoter will disqualify any entry from individuals who do not meet the eligibility requirements, and Promoter will also reject any entry knowingly received from persons under the age of 18.
d.    Listeners are eligible to win a prize in a Promotion conducted by the Station only once every six (6) months if the prize is valued over $600.
 
3.         ENTRY
a.         To participate in the Promotion, you may enter via the following method(s):
 
    Listen to KVOO each weekday beginning on September 21, 2012 and ending on November 30, 2012 at approximately 9:12am, 2:12pm, 5:12pm weekdays during the promotion dates for the announcement of the recorded cue to call. Upon hearing the cue to call, caller 98 (as announced by the recorded cue to call) to get through to the Station contest line at 918-460-9898 will have an opportunity to guess what the Black Box  sound is as played during the recorded cue to call. Each caller will only have one (1) guess as to what the sound is. If the caller does not correctly guess the sound, he or she will win a consolation prize (TBD) but not the Black Box sound pay-off. If the caller correctly guesses what the sound is, he or she will win prize and the next time the Black Box sound is announced on-air, it will be a new sound. At the time of their call, callers will be required to provide all information requested including their full name, complete address (including zip code), day and evening phone numbers and date of birth in order to be eligible to win. In the event that the selected caller is disconnected or is found to be ineligible, the next eligible caller that successfully makes it through on the call-in line and completes their call will be a winner. There is no limit to the number of times a listener may attempt to call in to win, but a listener may be a winner only once. There will be up to a total of no more than eleven (11) winners on-air during the Promotion dates. The Station is not responsible for telephone service outages, delays, busy signals, equipment malfunctions and any other technological difficulties that may prevent an individual from completing his/her telephone call. Due to delays in the KVOO online streaming of its broadcast signal, listeners to the online stream may not be able to participate in or may be disadvantaged in participating in on-air contests.
 
4.         PRIZES
a.         Up to eleven thousand dollars in grand prizes will be awarded. Each grand prize will be a minimum of $1000 and will grow by $100 with every incorrect correct. When correct guess is made winner shall receive the progressive amount of $1000 AND an additional $100 dollars for each incorrect guess from start of play (round.)

b.    All prizes may be picked up at the office of the Station at the address below. Winners will forfeit any prize or prize certificate not claimed within thirty (30) days of winning. Prizes will be awarded in the form a check issued by Journal Communications which will be mailed to the winner.  No checks will be awarded, processed or mailed until winner has claimed the prize at the Station, presented all required documentation, and signed all required releases and tax forms. Checks will then be mailed within approximately ninety (90) days. Station is not responsible for the safe arrival of prize check. Winner assumes all risk of loss.

b.     There is no substitution, transfer or cash equivalent for prizes, except that the Station may, at its sole discretion, substitute prizes or cash of comparable value. The prizes are expressly limited to the item(s) listed above and unless otherwise expressly specified, do not include taxes, gratuities or any other expenses. Other restrictions may apply.
 
5.         WINNER SELECTION AND NOTIFICATION
a.         Decisions of the Station management with respect to the Promotion are final.

b.     Up to eleven thousand dollars total will be awarded during the promotional period. The grand prize winners will be notified upon winning on air by the on-air personality.

c.     Odds of winning a grand prize depends upon the number and order of calls received.

d.     Winners must listen to the Station to win, but need not be present to win.

e.     The winners must execute and return any required affidavit of eligibility and/or liability/publicity release within ten (10) days of winning or prize will be forfeited. If a potential winner cannot be contacted, fails to sign and return the required affidavit of eligibility and/or liability/publicity release within the required time period, or if a prize or prize notification is returned as undeliverable, the potential winner forfeits the prizes.

f.           Promoter reserves the right to modify the rules at any time without prior notification.


6.         CONDITIONS
a.         Payments of all federal, state and local taxes are solely the responsibility of the winners.  Winners will be required to complete and submit an IRS Form W-9 or the equivalent including the winner’s full Social Security Number for receipt of any prize valued at $600 or more or for any prizes awarded by Promoter in a calendar year with an aggregate value of $600 or more. Failure to submit a complete W-9 or equivalent will result in disqualification and forfeiture of the prize.
b.         By participating in the Promotion, each winner agrees to have his or her name, city, state, voice, or likeness used in any advertising or broadcasting material relating to this Promotion without additional financial or other compensation, and, where legal, to sign a publicity release confirming such consent prior to acceptance of the prize.
c.         Prior to awarding any prize or prize certificate, and depending on the nature of the prize and eligibility requirements of the contest, the Promoter in its sole discretion will require verification of Promotion winner’s identification by a showing of valid government-issued photo identification.  By participating in the Promotion, each winner agrees to release hold Promoter, its sponsor(s) and promotional partner(s), and each of their parent, subsidiary and affiliated entities, and the officers, shareholders, directors, employees, agents and representatives of each of them (collectively, the “Released Parties”) harmless against any and all claims or liability arising directly or indirectly from the prize or participation in the Promotion.  Promoter will also require eligible Promotion winners and their guests or travel companions, if any (as well as each of their parent(s) or legal guardian(s), if winners, guests or travel companions are under the age of majority in their state of residence), to sign a liability release confirming such consent.
 
d.         Promoter, in its sole discretion, reserves the right to disqualify any person (and all of their entries) from this Promotion if he or she tampers with the entry process, the operation of the Promotion, or the operation of Promoter’s web channel or is otherwise in violation of the rules.  Promoter further reserves the right, at its sole discretion, to modify, cancel, terminate or suspend the Promotion, or any part of it, if it is not capable of completion as planned or if any fraud, technical failures or any factor beyond the Promoter’s control, including infection by computer virus, bugs, tampering, unauthorized intervention or technical failures of any sort, corrupt or affect the administration, security, fairness, integrity or proper conduct of the Promotion as determined by Promoter in is sole discretion.  Any attempt by an entrant or any person to deliberately damage any Promoter web channel or to undermine the legitimate operation of this Promotion may be a violation of criminal and civil laws.  Should such an attempt be made, Promoter reserves the right to seek full prosecution and/or damages from any such individual to the fullest extent permitted by law.  Promoter’s failure to enforce any term of these Official Rules shall not constitute a waiver of this provision.  If due to circumstances beyond the control of Promoter, any competition or prize-related event or travel is delayed, rescheduled, postponed or cancelled, Promoter reserves the right, but not the obligation, to modify, suspend or cancel the Promotion and shall not be required to award a substitute prize.
e.     The Station reserves the right to make changes in the rules of the Promotion, including, without limitation, the substitution of a prize of equivalent value, which will become effective upon announcement. If due to circumstances beyond the control of the Station, any competition or prize-related event or travel is delayed, rescheduled, postponed or cancelled, the Station reserves the right, but not the obligation, to cancel or modify the Promotion and shall not be required to award a substitute prize.

f.         The Released Parties are not responsible for (i) typographical or other errors in the printing, the offering or the administration of the Promotion, or in the announcement of a prize; (ii) entries not received due to difficulty accessing the internet, service outage or delays, computer difficulties, malfunctions, disconnections, other technological failures, telephone service outages, delays, dropped calls, or busy signals; or (iii) lost, stolen, mangled, misdelivered, postage due, illegible, incomplete, incorrect, or late entries.
g.          To obtain a copy of the Official Rules of the Promotion (please specify which), send a self-addressed, stamped envelope specifying “Official Rules” to FIAT 500 GIVEAWAY, KVOO 4590 E. 29th Street, Tulsa OK 74114.   A copy of the Official Rules is also available during regular business hours at KVOO 4590 East 29th Street, Tulsa, OK.  All such requests must be received within thirty (30) days following completion of the Promotion.
 
7.         TERMS OF USE AGREEMENT & PRIVACY POLICY
